20 Mar 2021 - 737-400SF, PK-YSF (23869/1639), FF 15/11/1988 (33 Years old), operated by Trigana Air Service suffered a runway excursion and main landing gear collapse whilst landing with one engine inoperative at WIHH, Jakarta Halim, Indonesia.

The follwoiing information is taken from the preliminary report issued 3 May 2021.

The aircraft departed Halim normallyat 10:52 LT with an assumed temperature of 40C. Shortly after takeoff the crew reported a problem with the RH engine with no fire. They held for troubleshooting and reported ready for an ILS approach to runway 24 at 11:16LT.

The aircraft weight for the approach was 131,500 lbs, while the maximum landing weight was 121,000 lbs.

The aircraft approached ILS Runway 24, with flap 15 configuration, autobrake 3, and the target threshold speed was 163 knots.

During the approach, the PM advised to the PF that the fuel imbalance reached the maximum limitation of 1,000 lbs.

The aircraft landed at 11:26LT. The final wind check was 060/6 (6kt tailwind).

Below 500ft RA the EGPWS alert GLIDESLOPE sounded 4 times and TOO LOW TERRAIN sounded 3 times.

The aircraft touched down 580m from the runway 24 threshold at 171kts CAS, the left engine thrust reverser deployed, the speed brake handle extended, and the vertical acceleration was 1.79 g.

The first scratch of the runway surface was found about 360 meters after the first right tire mark. The RH MLG collapsed and the aircraft skidded off the right side of the runway where all remaining gear collapsed. The tire marks of the left landing gear travelled off runway pavement about 2,050 meters and the aircraft stopped about 2,200 meters from the beginning of Runway 24.

There were no reported injuries. The Indonesian KNKT have opened an investigation.

Trigana Air has had 23 serious safety incidents since 1992, resulting in the loss of eleven aircraft and major damage to 11 others. At the moment there is no suggestion that this incident was any fault of the airline or crew.
